Provision of key workers to families to enable them to support the development of their child's potential. Service is provided utilising a transdisciplinary approach, where various professionals (allied health) are available to support families through their key workers. Support with inclusion in early education and care, kindergarten and school transition is provided, with information on generic services including respite, counselling and disability-specific services including accessing the NDIS. Children are seen in their homes and early childhood settings. NDIS registered.
