CCH has a multidisciplinary team specialising in the rehabilitation and assessment of the frail aged and people with disability living at home in the St George area. Services include: assessment; social work; podiatry; psychology; occupational therapy; rehabilitation; continence; nurses and equipment lending pool. ACAT assessments; Dementia day care; nursing home/hostel placement service; hydrotherapy; mobility groups and Brain Injury Day Centre.
Services may be appropriate for younger people with disabilities. Also has Transitional aged Care Team (nurse; physio and physio aid) for up to 18 weeks for people coming out of hospital.
