This non profit organisation specialises in assessment, care management and brokerage services for people with complex care and support needs, and for their families and carers. Programs include: transition options, independence at home (incl independent living skills training), disability options, (incl information, carer support, service access, visits at mealtimes and small group outings), ComPacks (a case-managed package of care for up to 6 weeks after discharge from hospital), Case Management for people with long-term disabilities and Healthy at Home Program (Dept of Health funded). Also Sub Acute Fast Track Elderly (SAFTE) to help elderly people stay at home by receiving same care as ComPacks.
