Provides a wide range of in home and community based services to older people, people with disabilities and their families. Services include personal care, social support, community access, domestic assistance, case management and coordination. A number of both short and long term programs are available. Eligibility: HACC clients. Registered NDIS provider focusing mainly on plan management and coordination.
