Northern Sydney Home Nursing Services provides Chronic/Complex Care Coordination and Intergrated Chronic Care for People with Chronic Diseases by: Comprehensive clinical assessment; Identifies social determinants of health that impact on self management of chronic disease; Coordination of and referral to clinical and non-clinical services; Shared Care Plans - collaborates with patients, carers, family, GP, specialist services, allied health and NGO's; Health Advocacy - facilitates access to healthcare in the community; Health Coaching - identify enablers and barriers to self-manage chronic conditions, promote healthy behaviour and achievement of health-related goals.

Aged 16 years and over and have at least one of the following diseases: Chronic Obstructive Pulmonary Disease; Coronary Artery Disease; Hypertension; Congestive Heart Failure; Diabetes and identify as high or very high risk of unplanned presentation
